D R TA V A K O L I . C O M . A U YOUR GUIDE TO RHINOPLASTY DR KOUROSH TAVAKOLI EXPLAINS WHAT TO EXPECT BEFORE DURING AND AFTER A RHINOPLASTY PROCEDURE. DR KOUROSH TAVAKOLI riginating from the Greek Rhinos meaning nose and Plassein meaning to shape rhinoplasty refers to cosmetic surgery of the nose and is one of the most common of all plastic surgery operations. In fact rhinoplasty was among the first cosmetic plastic surgery operations ever performed way back in 1898. The current Australian trend in rhinoplasty is all about the natural look. However it is highly individualised surgery and you should discuss your individual desires with your chosen surgeon during consultation. O Who is a suitable candidate Thebestcandidatesforrhinoplastyarethosepeoplewhoarelookingforanimprovement in the way they look not those searching for absolute perfection. In fact patients who are completely obsessed with the appearance of their nose to the extent that it interferes with their daily living should perhaps seek counselling prior to consulting a plastic surgeon. Well-meaning friends and family members are not a good source of information. You should thoroughly research the procedure before proceeding with surgery a well-informed patient is a happy patient. Rhinoplasty surgery when performed on the right candidate by the right surgeon can produce a profoundly positive transformation to one s face making the person more attractive and in most cases look younger. Rhinoplasty usually takes around two hours to perform although more complicated procedures may take longer. The rhinoplasty surgeon Rhinoplasty is one of the most difficult operations in cosmetic surgery. In Australia rhinoplasty is performed by five groups of doctors therefore your decisionmaking process should heavily rely on word of mouth and personal research. Choosing a surgeon specifically for rhinoplasty is highly advisable. Plastic Surgeons (usually for cosmetic reasons) Ear Nose and Throat Surgeons (usually for functional reasons) Others Maxillofacial Surgeons General Surgeons General Practitioners Before the rhinoplasty procedure Test required In order to get the most from the initial consultation a CT scan (not an x-ray) of your nose and sinuses should be arranged. Your GP should be able to organise this especially if you suffer from any of these conditions Breathing problems Previous injury to the nose Previous surgery of the nose Hayfever or sinusitis. W W W. D R TA V A K O L I . C O M . A U DR KOUROSH TAVAKOLI Preoperative consultation This is where you need to convey your aesthetic wishes and goals clearly to your surgeon. A great deal of time will be needed to establish a rapport so that all aspects of rhinoplasty surgery can be thoroughly discussed. I prefer a minimum of two consultations prior to surgery and in some special cases three or four visits may be needed. My practice has the latest digital imaging system available and together we can communicate to each other about your desired cosmetic goals via a large computer screen. Although useful the imaging system should be used only as a guide and not a guarantee of outcome. The rhinoplasty operation Cosmetic and functional surgery (rhino-septoplasty) of the nose usually takes approximately two hours and for your safety it is performed under a general anaesthetic in a fully accredited day surgery or major hospital. Most incisions are placed inside the nostrils (known as a closed rhinoplasty) but a small incision may be made in the columella (known as an open rhinoplasty). The columella is the skin dividing the nostrils. In my experience these incisions tend to heal quickly and leave scars that are nearly invisible. Rhinoplasty surgery starts with the skin of the nose being separated from its supporting framework of the bone and cartilage which is then sculpted to the desired shape. The skin is then re-draped over the new framework. Septoplasty is performed if needed to straighten any deviation in the septum and or harvest cartilage graft for nasal re-shaping. Turbinoplasty is performed if these air-filtering structures are unusually enlarged. At the end of the rhinoplasty operation an aluminium or thermoplastic cast will be placed on your nose. The external splint will stay on for around a week. Usually a light dressing packing may be placed inside the nostrils and removed the same day or the next day. If septoplasty is also performed for airway correction a plastic tube may be inserted in the nostrils for up to three days post-procedure. Recovery from the rhinoplasty procedure During the first two to three days after rhinoplasty surgery your face will feel puffy your nose may ache and you may experience a dull headache. You should plan on resting with your head elevated for the first couple of days. There will be swelling and bruising around the eyes reaching a peak after around four days. By the end of week one all nasal packing dressings internal and external splints and stitches should be removed. You will feel substantially better in your second week of recovery. The most uncomfortable part of the recovery after the rhinoplasty operation is typically the inability to breathe through your nose for several days due to the swelling. This usually improves within around four weeks. You should also expect some swelling around your eyes for several weeks. As the swelling subsides you will start to see the improvements from your surgery. W W W. D R TA V A K O L I . C O M . A U DR KOUROSH TAVAKOLI Post-surgery precautions You will need to take a few special precautions during your recovery from a rhinoplasty operation. Bending or lifting can prolong swelling so this is not recommended for at least three weeks after surgery. Running cycling or aerobic activity should not be resumed for at least four weeks. Contact sports such as hockey soccer and basketball should be avoided for eight weeks as it takes that long for the bones to fully heal. You may drive when the swelling subsides enough not to interfere with vision and you may fly two to three weeks after surgery. Follow-up visits are required as needed for 12 months after surgery. WHAT CAN A RHINOPLASTY ACHIEVE Remove humps over the bridge of the nose Modify and refine the nasal tip Improve a crooked nose by re-aligning the nasal bones Improve the angle between the nose and the upper lip Shorten or lengthen the length of the nose Increase or decrease nasal tip projection Narrow the nostrils Immediate results Nose bleed Mild bleeding is expected. Moderate bleeding is usually seen in septoplasty cases. This can be managed by keeping your head up and spraying Drixine nasal spray. If you experience severe bleeding I have an emergency mobile number to be contacted on present to any hospital emergency department. Mild infection This can occur despite oral antibiotics and is more common in secondary rhinoplasty cases. Please ensure your nostrils are kept impeccably clean and scab-free and that you are using the prescribed antibiotic ointment on a regular basis. Breathing issues This is a very common temporary issue after a rhinoplasty procedure. It is usually a result of swelling and congestion of the internal lining of the nose. Keep up the use of FESS (salt water) spray duringthe first month after the surgery and breathing shouldimprove within two months. Possible long-term results WHAT CAN A SEPTOPLASTY & TURBINOPLASTY ACHIEVE Straighten a deviated septum and thereby improving breathing Reduce the bulk of air filters (turbinates) improving breathing further Prolonged swelling of the tip of the nose six to 12 months post-surgery. This is not a complication but a variable prolonged response to surgical inflammation and trauma. This is far more common in men and women with thick oily (sebaceous) skin. Nostril size discrepancy that is present prior to rhinoplasty may become more evident once the nasal tip is lifted. Achieving perfect nostrils may not always be possible. If you are having a crooked (bent) nose corrected at the time of your rhinoplasty please note that making such noses completely straight is impossible. Most people have some degree of bend in their noses which goes unnoticed. Secondary or revisional surgery may need to be performed in order to finalise the outcome. This is far more common in men and women with thick oily (sebaceous) skin. With careful planning and meticulous aftercare rhinoplasty surgery can have a significant impact not only on the appearance but also on the function of the nose with results that can last a lifetime. A U DR KOUROSH TAVAKOLI MAKEOVER MARVEL A PATIENT DESCRIBES THE PROCESS SHE UNDERWENT WHEN DR KOUROSH TAVAKOLI PERFORMED A COMBINATION OF PROCEDURES TO GIVE HER A FRESH NEW LOOK. ail is a 51-year-old operating theatre nurse with two small children. She decided some time ago that she was unhappy with the overall appearance of her face. After careful reflection and much research she went to see Sydney plastic surgeon Dr Kourosh Tavakoli. I d been thinking about getting something done for some time says Gail. I didn t get married until I was 40 years old and so had my first child at 40 and my second at 43. There were a few things about my face I just didn t like especially around my mouth and jaw line my jowls were sagging a bit and I ve always hated my nose she explains. Many patients opt to undergo one procedure at a time but as a veteran theatre nurse Gail was experienced enough to know how to do her research and confident enough to undergo all the procedures she wanted at once. I thought if I m having an operation on my face I might as well have everything I want done at the same time she says. Gail began her search for the right surgeon by asking her colleagues for advice. I knew a few doctors and I asked around for their recommendations she says. Then I looked up each doctor s website and narrowed it down to just two then saw both for a consultation. I chose Dr Tavakoli because he has a very good reputation. Nine months after her initial consultation she returned to Dr Tavakoli and booked herself in for the surgery. Dr Tavakoli performed a six-hour operation to rejuvenate her face and refine her nose comprising a short scar facelift upper and lower blepharoplasty rhinoplasty lip augmentation and finishing with Botox injections in her forehead. Gail says she was very bruised immediately after the procedures but her profession had prepared her to expect this as a first result. It s now been five months since her surgery. She says an operation of this scope was bound to take some time to recover from and though the recovery process was a gradual one it was well worth it. When I look at myself now I m really pleased with the result she says. However it wasn t until she saw Dr Tavakoli s final set of after photos that she realised the full extent of the dramatic changes to her appearance. People I haven t seen in a long time know there s something different but can t quite put their finger on what s changed. That s a good thing I didn t want to look like someone else I still wanted to look like me but maybe better or younger and healthier Gail says. Dr Tavakoli s work on my nose was very subtle but I really like the result. A U DR KOUROSH TAVAKOLI MY PROCEDURE A PATIENT SHARES HER EXPERIENCE BEFORE DURING AND AFTER HER RHINOPLASTY PROCEDURE PERFORMED BY DR KOUROSH TAVAKOLI. melia had been thinking about undergoing cosmetic surgery to change the appearance of her nose for a while but wanted the result to be subtle and perfectly suited to her face. My nose always bothered me she says. I used to look at photos of myself and wish I could get rid of a specific bump and make a part of the bridge narrower but I didn t think a surgeon would be receptive to such small changes. After thoroughly researching the risks recovery and operation itself Amelia and her best friend spent time going to consultations with different surgeons to receive several opinions. It was extremely helpful to sit with a number of surgeons and discuss my options and it allowed me to refine the criteria of what I was looking for in a doctor she says. The initial research process was made easier with the support of her best friend who was also interested in undergoing a rhinoplasty procedure. Together they discussed their individual concerns in depth and were able to clearly define their desired results. My nose was quite asymmetrical because my septum was deviated which not only looked crooked on the outside but also made breathing out of my right nostril quite difficult Amelia explains. I also wanted the bridge of my nose near my eyes to be narrower my nostrils to be slightly smaller and the tip of my nose to be less rounded. I wanted all of these changes but for them to look natural and subtle something I was scared wouldn t be possible. Amelia chose to follow through with Dr Tavakoli who she says was very understanding of her concerns and made it clear to her that he understood what she wanted. I was confident Dr Tavakoli knew exactly what I was after she says. He showed me mock digital images of what my nose would most likely look like after surgery and explained to me exactly what needed to be done to achieve it. It was very important to me that my surgeon and I were on the same page. Amelia says she felt comfortable asking all kinds of questions during the entire process from consultation to recovery. Dr Tavakoli and his staff were so happy to answer all my questions no matter how silly they seemed. Because she had thoroughly researched what to expect post-procedure she was prepared for what the recovery period involved. It went really well. When I first woke up after the operation I felt quite drowsy and had to take lots of short naps. Dr Tavakoli and his nurses kept checking on me and at times I felt a bit confused as a result of the general anaesthetic but this passed and I was moved into a big comfy chair in the recovery area. Someone was with me every step of the way so I felt really cared for and in safe hands she says. Dr Tavakoli also gave me information sheets that clearly described what I would go through on Day One and Week One after the procedure she continues. My eyes were swollen for two days and I was bruised under one eye. After this I was able to walk around and do normal things but I still had some pain left over so I took it easy for about a week. It s now been six weeks since the operation and I m back to normal life it s great Her results I couldn t be happier with my new nose People don t usually pinpoint I v e had a rhinoplasty but they do comment on how refreshed I look. A U DR KOUROSH TAVAKOLI RHINOPLASTY CASE STUDY R A RHINOPLASTY PROCEDURE BY DR KOUROSH TAVAKOLI TRANSFORMED THIS PATIENT S APPEARANCE AND LIFE. hinoplasty is one of the most common cosmetic surgery procedures performed in Australia and one with a high patient satisfaction rate. According to Sydney plastic surgeon Dr Kourosh Tavakoli many people take a long time to actually decide to have the surgery but once they do many admit they wish they had done it sooner. At 20 years old Dr Tavakoli s patient says that it was the perfect time to correct her nose that she had been very aware of throughout her life. Being young I had no big commitments and I knew the healing time would be relatively quick she says. I just didn t want to wait any longer. The patient says she has never been happy with her nose and always wanted to change it. It just didn t look feminine and was out of place on the rest of my face. I had the misfortune of inheriting my father s masculine European nose she says. It just didn t fit with the rest of my facial features. There was a prominent bump which was especially noticeable in my profile. She began researching rhinoplasty procedures reading about other people s experiences online and found Dr Tavakoli s case studies. Although she spoke to other doctors I was impressed with Dr Tavakoli s qualifications and professionalism she says. She also spoke to some of his former patients. In her first consultation Dr Tavakoli discussed the surgical possibilities techniques and her desired outcome. This was extremely beneficial as seeing the after shots meant I had realistic expectations of what could be achieved. Dr Tavakoli also detected some breathing problems in the patient s nasal passages. It turned out my left nasal cavity was completely blocked off so the procedure wasn t just cosmetic after all she says. The patient says her recovery was relatively slow but Dr Tavakoli and his staff were very helpful with post-surgery care. I was one of the small percentage of people who was still experiencing bruising and swelling at my one-week followup appointment she says. I had lymphatic massage to drain the build-up in my face which cleared it up quite quickly. The patient says once the swelling subsided she was absolutely delighted with the result and her breathing had improved. It was exactly what I expected and hoped for she says a very dramatic change. C osmetic surgery is often something people wish to do for a long time but put off says Sydney plastic surgeon Dr Kourosh Tavakoli. They may postpone for months or even years perhaps through selfconsciousness anxiety or lack of self-confidence. However once the procedure is completed most patients say they wish they hadn t waited so long. Sarah was one such patient. Sarah came to Dr Tavakoli with aesthetic concerns about her nose as well as functional difficulties she wished to have corrected. My nose had always bothered me because it was bigger then I would have liked she explains. I also had breathing problems due to a deviated septum. Dr Tavakoli guided Sarah through the consultations and they worked up a computer image of her proposed nose based on their discussions. Th e procedure itself was so easy. All I remember is laying down on the surgery table then waking up when it was over says Sarah. Th e recovery was a lot better then I had expected. I didn t have much pain or bruising at all but it was a little hard breathing through my nose for a week after surgery although I knew this was to be expected. I took about three weeks off work to recover fully before my return. Sarah s feelings on the results are very positive. Dr Tavakoli was great. I couldn t have been happier the final result was actually better than I imagined she says. People have even said my new nose suits my face more than my previous nose and looks very natural. I was extremely self-conscious especially camera shy as I hated my nose being captured in profile she says. I didn t like the look of my nose and felt it didn t match or complement the rest of my face. Jodie felt apprehensive during her first consultation but says Dr Tavakoli was reassuring and made her feel at ease. It is a big deal putting your trust in someone else s hands but Dr Tavakoli calmed my nerves by answering all my questions and explaining the procedure carefully she says. We had a talk about my concerns then after he did an examination we looked through pictures of his previous work and mocked up digital images of how my new nose would look. Waking up after the procedure Jodie says Dr Tavakoli visited her and reassured her that everything had gone well. I hadn t seen my nose because of the splint you have to wear for several days following the procedure she says. Once the swelling and bruising began to go down my new nose started to emerge and I could see it was exactly what I wanted. Dr Tavakoli removed the bump on the bridge of Jodie s nose shorted the tip and reshaped the nostrils. It is now what I would describe as a button nose it s more feminine and enhances my face and other features so everything looks in proportion says Jodie. What Dr Tavakoli did with my nose was amazing. He s a talented surgeon and I would go back to him for future procedures. Jodie took a month s holiday after her procedure but says after five to seven days she was feeling well enough to leave the house. With the painkillers and antibiotics it was a very manageable recovery she says. And the results I love my results It was the best decision I ve ever made and I can t believe I didn t do it sooner she says. My friends and family can see my confidence has grown. C atherine s nose was broken when she was four years old. At the age of 22 and tired of daily headaches and breathing difficulties Kelly sought the advice of Sydney plastic surgeon Dr Kourosh Tavakoli. I had a bump on my nose which I found unattractive but even more disturbing were the constant headaches every day she says. My nose always felt as if it were blocked but I had become accustomed to it. Dr Tavakoli took x-rays and it was obvious the passages inside my nose were extremely small so it wasn t surprising I had trouble breathing. He advised me to undergo a rhinoseptoplasty which is a combination of a cosmetic rhinoplasty to correct the bump and a procedure to deal with the airway issues. Catherine says that using computer imaging Dr Tavakoli showed her what her n ew nose would most likely look like. She was able to participate in the choice of nose shape deciding whether she wanted a dramatic or subtle change to create a natural-looking nose that sat well with her other facial features. Under a general anaesthetic in hospital Dr Tavakoli performed an open rhinoplasty and found extensive damage in the airways especially on the left side of her nose. He inserted some artificial cartilage in the upper part of her nose where the damage had occurred. In a closed rhinoplasty most incisions are placed inside the nostrils however in an open rhinoplasty small incisions are made in the skin dividing the nostrils. These incisions heal quickly and leave scars that are nearly invisible. Septoplasty is performed to straighten any deviation in the septum. When I woke up I was bandaged and had a small splint on my nose she says. I also had tubes inside my nose to assist with breathing due to swelling. These were a bit uncomfortable. After seven days the splint tubes and bandages were removed along with the stitches inside my nose and around the outside of my nostrils. My nose was swollen and I had some bruising under my eyes but it wasn t as bad as I had expected. The bruising cleared after about two weeks and the puffiness took a little longer although I think I noticed it more than a casual observer would she says. I m very happy with the appearance of my nose but the dramatic improvement in my breathing is the main benefit. I have no more headaches and my breathing is a thousand times better. R hinoplasty when performed on the right candidate by the right surgeon can have a profoundly positive impact on a person s face often making them look more attractive and in most cases younger. Recently engaged Bronwyn who is now 29 consulted with Sydney plastic surgeon Dr Kourosh Tavakoli in 2011 in the hope he would be able to improve the appearance of her nose before her wedding. My nose had quite a large bulbous tip and it was bent to the left she recalls. I d thought about having surgery for a few years but bit the bullet when my partner proposed. Bronwyn s nose had never caused her any great emotional distress. However she was conscious of how her nose appeared in certain angles when being photographed and once she had secured the financial means it became something she wanted to change. A U DR KOUROSH TAVAKOLI Bronwyn s healthy motivation for seeking rhinoplasty meant she had realistic expectations and was therefore an ideal candidate for rhinoplasty. I was not expecting a complete change just an improvement on what I already had she says. Dr Tavakoli spent a lot of time talking to me during two consultations before I went ahead with the operation. He agreed with me on almost all my ideas and we went through a book of his previous work and together worked out what would be a good outcome for me. Although Bronwyn went to Dr Tavakoli for aesthetic concerns it turned out the rhinoplasty was going to be beneficial for functional reasons too. Once the CAT scans were returned it was found that I had restricted use of both sides of my nasal passage she explains. Bronwyn underwent surgery in September last year. Whilst her fianc e was initially very worried he quickly accepted Bronwyn s decision but she also faced questions from friends and family members asking why she felt she needed the operation. The reality was that I was making a choice for myself not for anyone else Bronwyn says. And regardless of what anyone else thought I needed I believe the best person to assess what you want and need is yourself. Bronwyn says her recovery was a little uncomfortable at first but she benefitted from having her fianc e on hand to take good care of her post-op. He made sure I took my medication on time and that I had cold compresses every 30 minutes she recalls. I was a bit groggy after waking up from the anaesthetic and it was slightly uncomfortable because I couldn t breathe through my nose with all the packing and so forth. I was quite swollen but there was little bruising and almost no pain. After three weeks most of the swelling had reduced and Bronwyn was able to return to work. For six weeks she was not permitted to undertake activity of any kind that would raise her blood pressure. Thereafter Bronwyn was able to return to the gym but not participate in any contact sports or anything that involved bending over which would send blood rushing to the head. Bronwyn recalls the moment she saw her new nose. Even though my nose was still quite swollen when I had the splint taken off after seven days I cried because I loved my new nose so much she says. I was so happy. The first thing I said was I love it I think the staff were a bit shocked as they must experience some people who are a little unsure because of the swelling in the early days. Nearly a year on Bronwyn says her new nose is everything she expected. Dr Tavakoli is an artist. But as well as the aesthetic improvement my breathing is so much better now she says. Bronwyn says she is amused that people she chose not to tell about the surgery even members of her own family didn t realise that she had had anything done. You could get away with not telling anyone and they probably wont even notice she says. For me that s a good thing. Even though he was unsure about my decision originally my fianc e tells me that I look so much better and it has really improved my overall appearance. T he number of breast enlargement operations is on the increase and the demand for natural-looking breasts is even more apparent. As a plastic surgeon specialising in breast augmentation and breast lifting procedures I have been privileged to treat a large number of patients every year in my Sydney clinics. Therefore I have drawn from this wide experience to perfect my formula for successful attractive and natural-looking breast augmentation. Natural-looking breasts philosophy A well-augmented breast has a natural fullness with gentle sloping off the chest wall. There should be natural cleavage without webbing between the breasts and only a certain amount of perkiness. There are three main telltale signs of unnatural-looking breast augmentation 1. Breast implant margin This should be soft and imperceptible as obvious implant edge visibility especially in the midline will resemble the Tori Spelling or Posh Spice look. 2. Breast implant size An implant too large for a small frame can be an obvious giveaway a patient has had breast augmentation. The best example of this is Pamela Anderson-style breasts. Women wanting to go from an A to a full D cup need to understand this point. 3. Perkiness An augmented breast that is too perky will tend to look fake as a small amount of droop is natural. When a patient lies on her back the breast implants should roll to the side like natural breast tissue and not sit up like rigid peaks. Before undergoing surgery it is beneficial for the patient to review photographic examples of the breasts they would like and to convey their wants to the surgeon so there is a clear visual understanding of the desired result. Please note these photos can only be used as a mere guide. Essential clinical parameters There are six main clinical parameters for breast implants on initial consultation 1. Patient s body build and height 2. Patient s own breast size shape and symmetry 3. Nipple position in relation to the breast mound 4. The quality of breast skin (thickness and stretch marks) 5. Chest wall shape and dimension (hollowed vs pigeon chest walls) 6. Patient s desire for new cup size under-proportion in-proportion or overproportion augmentation. After establishing the crucial parameters in the clinical examination the process of tailor-making the correct breast augmentation begins. Tailor-made breast enhancement I believe there are six key considerations for a surgeon when customising a breast augmentation to gain a natural-looking result 1. Incision placement 2. Incision placement There are three choices about where to make the skin incisions for breast enlargement. They can be in the breast fold (inframammary) around the nipple (periareolar) or underneath the armpit (transaxillary). These incisions can all produce scarring which can range from excellent to poor. Although patients may voice some initial concerns about the location of their scars they are ultimately far more concerned with the final shape and size of their breasts. In fact the rate of scar revision for unsatisfactory scarring in breast augmentation is less than 0.05 percent. Generally a great majority of patients in my practice opt for the inframammary incision (breast fold). I also find this incision has the least amount of interference with breastfeeding and nipple sensation and it generally heals very well. I find the periareolar incision particularly useful in some Asian and African patients with higher risk of keloid scarring but the nipple-areola must be at least 4cm in diameter. Furthermore the periareolar incision also allows me to perform the full Benelli breast lift or nipple lift where this may be indicated in mildly droopy breasts. Transaxillary (armpit) incision carries a higher risk of cleavage problems (too much gap) and is reserved for a few select patients with a small chest wall. The transumbilical (belly button) incision popularised by the Fox reality show Dr 90210 is only reserved for the use of saline implants. As I am not a huge fan of saline implants due to the extremely high risk of problematic rippling I do not personally favour the belly button incision. My clinic offers Fraxel laser technology. This fractionated laser works significantly well in most scars to eliminate the redness and improve texture of scars in cosmetic surgery patients with poor healing. 2. Breast implant shape The choice of implant varies from round to teardrop shapes. The round implant comes in both low- and high-profile varieties. The shape variation is in the width and projection of the implant for any given size. The spectrum of breast implants available to the surgeon can therefore provide great versatility in achieving a more natural look. The majority of patients in my practice elect to have round-shaped implants. Since I prefer placing the implant in a submuscular pocket in most patients implant edge visibility in the upper pole of the breast can be avoided. The round implant tends to be ideal for those patients with well-shaped natural breasts who desire a straightforward enlargement. Use of the teardrop (anatomical) shape depends on the patient s desire as well as her breast shape. In general there are two groups of women who benefit from teardrop-shaped breast implants. Firstly it can be the ideal choice for women who have mild droopy and or tuberous breasts. Mild elevation of the nipple in relation to the breast mound can be achieved without the need for extra scars on the actual breast (unlike breast lift scars). In these cases the implants will be inserted in a subglandular or subfascial pocket (under the breast tissue). In moderate to severe cases of droopiness a W W W. D R TA V A K O L I . C O M . A U DR KOUROSH TAVAKOLI breast lift must be performed at the same time as breast augmentation in order to restore aesthetic shape. Secondly some patients specifically want less fullness in the upper quadrant. Teardrop breast implants certainly offer less fullness in these particular situations. It should be noted however that this request tends to be very personal as most women are seeking breast augmentation to obtain upper pole fullness. It should also be noted that teardrop or anatomical implants do have a tendency estimated at about five percent to rotate. Unfortunately this problem can only be corrected by secondary surgery. The rate of teardrop implant rotation in women who continue to use the chest muscles in their exercise routine is much higher. Round implants by the very nature of their shape do not cause rotation issues. 3. Breast implant size Attractive breast augmentation should be in proportion or slightly out of proportion to the woman s overall body shape Breast implant size is one of the most important decisions in a breast augmentation procedure. Because of this a good surgeon will take several approaches to help the patient make the best decision based on her anatomy personal preferences and the appearance she wishes to achieve. In general attractive breast augmentation should be in proportion or slightly out of proportion to the woman s overall body shape. In a recent survey over 80 percent of patients undergoing breast augmentation stated an average to full C-cup was their desired postoperative goal. D-cup was the second most popular request. Small C-cup is the third commonest followed by full-D and DD cups. Final breast implant size is a complex function of the elasticity of a patient s skin envelope chest wall diameter and implant dimensions but most importantly pre-existing breast volume. For this consideration I always begin by showing the patient numerous photos of actual patients who have had breast augmentation. By finding someone who has a similar preoperative appearance and evaluating their results with the size of implant they chose the patient can get an idea of her own final result and increase or decrease the implant size according to her wishes. I also measure the patient s breast and chest shape paying particular attention to the base and projection of the breasts. This gives me an idea of what size breast implant will help achieve the patient s desired size. As breast implant size increases so does the diameter of the breast implant. In most cases there is a breast implant that will be an ideal match for the diameter of the patient s natural breast and I find this is a good starting point for discussion. Choosing a breast implant smaller than the patient s natural breast shape will not provide the proper cleavage and shape following the procedure. Similarly choosing a breast implant too large for the patient s natural chest shape is more likely to give an unnatural appearance. It is noteworthy that very large implants can create more issues and future complications such as stretch marks implant migration and symmastia. Unfortunately breast implants do not come in cup sizes. Rather they are categorised by the volume of gel they are designed to hold. There are several reasons for this. First the final cup size will be partially determined by the preoperative breast size and every patient is different in this regard. Second a C-cup from one bra manufacturer is not necessarily the same as a C-cup from another. Although every woman is built differently and bras are not to a set standard W W W. D R TA V A K O L I . C O M . A U DR KOUROSH TAVAKOLI The surgeon must be skilled in deciding the best option for each individual patient performing the actual surgery and offering meticulous follow-up it can be expected that a B-cup implant size is approximately 250g (cc) and a C-cup is 330cc in a woman of average height and average build. That number will be higher if the woman is tall or has broad shoulders. Similarly if the patient is shorter than average or has a narrower chest that number can be expected to be slightly lower. Although a desire for a certain cup size is helpful in guiding the patient in the selection of the proper breast implants I find it is more helpful to focus on the desired shape and appearance the patient wishes to achieve. 4. Pocket position The next consideration is where to place the breast implant on top of or behind the muscle. In general I prefer to place breast implants behind the muscle so they are partially covered. I find the pectoralis muscle allows a smooth takeoff from the chest wall. If put directly on top of the muscle the breasts can look like rounded balls on the chest which is a definite giveaway. It is also my opinion that placing the implant under the muscle will in the long run have an impact on breast droopiness. Also for older women that need breast screening it seems that behind the muscle placement is superior from a mammogram and ultrasound imaging point of view. A further advantage of the submuscular pocket is a lower rate of capsular contracture. It is not all perfect with the submuscular placement of implants. The negatives of the submuscular pocket is that it may create winking on animation or flexing of the pectoralis muscle. Implant migration and displacement is also more likely in this pocket. In women with mild droopy (ptotic) breasts I use the dual-pocket technique of dissecting both on top of and underneath the pectoral muscle but inserting the breast implant behind the muscle. This new technique popularised by Texan plastic surgeon Dr John Tebbett is proving very successful with most types of breast augmentation that I see in my practice. In rare cases of moderate droopy breasts where the patient does not wish to undergo a breast lift procedure for fear of scarring I will consider placement of the implant on top of the muscle. This pocket is referred to in the plastic surgery literature as subglandular or subfascial . Generally speaking it is a relatively painless pocket with excellent short-term benefits but in my opinion far less long-term advantages compared with the submuscular or subpectoral pocket placement. 5. Implant coating This issue of different coatings is controversial. Implant surfaces can be generally smooth textured or polyurethane. The reason for the invention of differing implant surfaces has been to create the perfect implant that has a low capsular contracture rate and yet feels soft and natural. No matter what the coating on the outside of the implants is the inside material of all the implants I use are still silicone gel. The research in implant technology is ongoing and new advances are continuously emerging. Generally speaking textured or rough surface implants are said to reduce the rate of capsular hardening or contracture and have a lower rate of migration but they are also known to create more wrinkling and rippling issues later on. This wrinkling is normally felt in the lower edge of the breast where the implant is W W W. D R TA V A K O L I . C O M . A U DR KOUROSH TAVAKOLI closest to the skin surface. Smooth implants may give a smoother look and softer feel in many cases particularly in thinner patients but the downside is that the patient needs to massage the implants for three months to help prevent capsular hardening. My recommendation is to use textured implants in those patients who are predisposed to capsulisation and also for extremely athletic girls who may shift the implant pocket due to exercise over-exertion. Going forward polyurethane implants are a viable alternative to textured implants. Polyurethane coated implants from the manufacturer Silimed (Brazilian implant) are proving to be very effective in the prevention and treatment of capsular contracture. Currently I prefer to use these implants in selected primary cases and all revisional cases of capsular contracture. Please note that the Brazilian implant also has silicone gel in the centre of the implant and is TGA product approved here in Australia. 6. Implant fill I use very commonly the cohesive silicone-gel and rarely the saline-filled breast implants. The new generation silicone-gel breast implant is very safe and generally feels and looks more like a natural breast. Most breast augmentation operations in Australia are performed with siliconegel breast implants (98 percent). In December 2006 the United States Food and Drug Administration approved the use of gel implants in the US market. The decision was based on a great amount of scientific research into the safety of silicone-gel implants. As a result the use of gel implants worldwide is 90 percent. The gel usually comes in low and high cohesiveness (soft touch or firm touch) and patients should ask their surgeon for different samples at the time of the consultation so they can make the most informed choice possible. Generally speaking the firmer gel implants are form responsive like the gel found in the teardrop implants. The round implants can be soft (80 percent fill) or firm (100 percent fill) depending on the manufacturer. F or many women having a baby takes a toll on their body and the shape they are left with post-pregnancy can be a cause of concern affecting selfesteem and confidence levels. In a study by United Kingdom Mother & Baby magazine 86 percent of women surveyed said they felt less attractive postpregnancy while a similar number were unhappy with their weight and shocked by the changes to their body after having a baby. Yummy mummy celebrities such as Katie Holmes Heidi Klum and Jennifer Garner seem to regain their post-baby figures in double-quick time and now it appears that here in Australia women are equally determined to take steps to restore both their pre-baby body and confidence. In fact a large number are choosing cosmetic surgery to help restore their shape and are taking the opportunity to correct long-standing problem areas which may have become more prominent during pregnancy. Sydney plastic surgeon Dr Kourosh Tavakoli estimates up to 30 percent of his female patients have come to see him because they want to improve their bodies after childbirth. All of these women he says have a specific goal in mind and somewhat surprisingly it is not one that involves an idealised new body or perfect physique. Their aim is restoration not transformation. They have 50 or 60 years ahead of them and want to keep looking their best. There are numerous options available to women who want to tackle postpregnancy problems such as excess fat and skin around the abdomen and drooping sagging breasts. Troublesome areas that have caused some concern before pregnancy such as love handles can be exacerbated after giving birth. However these issues can be addressed with surgery and according to Dr Tavakoli the top three procedures requested by mothers who come to see him are breast enhancement abdominoplasty and liposculpture. Breast enhancement The degree of change experienced in the breasts depends on whether patients have breastfed and if so for how long. The more a woman has breastfed the more dramatic the changes that occur in her breasts usually are says Dr Tavakoli. Most women opt for a breast augmentation procedure but if there is no change to the volume of the breast a breast lift might be enough to achieve the result they want. However other woman may require both a breast reduction and lift procedure to obtain their desired shape he says. Abdominoplasty According to Dr Tavakoli the abdomen is the area most affected by pregnancy as the abdominal wall becomes lax due to stretching. An abdominoplasty procedure also known as a tummy tuck can restore a woman s post-pregnancy tummy and the procedure can be combined with liposculpture to offer an even better result. Abdominoplasty when combined with liposculpture can remove excess skin and fat from the abdomen repair any underlying damage to the abdominal muscles and reposition the umbilicus says Dr Tavakoli. Liposculpture As well as being effective when combined with abdominoplasty liposculpture can be used alone to treat common problem areas such as love handles and inner and outer thighs. As Dr Tavakoli explains These could be areas that troubled the patient prior to pregnancy but they may have become more prominent postpregnancy. The most important thing according to him is that the treated area remains in proportion with the rest of the patient s body. While these procedures can offer great results for women who want to restore their pre-pregnancy body Dr Tavakoli stresses that those who undergo an abdominoplasty or liposculpture procedure need to commit to a healthy lifestyle involving a sensible diet and exercise regime to maintain the results. He screens prospective patients to ensure they are not suffering from postnatal depression and has a policy of not operating on recently pregnant women until the mother s last child is at least 18 months old. I usually advise patients to complete all their pregnancies before commencing surgery he says. As long as the expectations are realistic I find the overall satisfaction rate in this group of patients is very high. It s clear that for some women who are unhappy with their post-pregnancy appearance a cosmetic surgery procedure could offer the ideal way to restore their figure and confidence levels. W W W.
